Criteria:

• Your card must be in good condition
  
• Your card must be in date and not expired
  
• The first 6 and last 4 digits of your card are the only digits visible in your photo
  
• Your photo is clear and is not obscured by any lighting
• Your full name is visible on your card.
  
• The name on your card matches the name you have listed on your account.
• In the case of any cards that are only initialed (Example A.B Smith) can be approved along with a copy of your bank statement.
